At Counterpart Health (CPH), we are transforming healthcare and improving patient care with our innovative primary care tool, Counterpart Assistant. By supporting Primary Care Physicians (PCPs), we are able to deliver improved outcomes to our patients at a lower cost through early diagnosis and longitudinal care management of chronic conditions.  

The CPH Program Manager will own the performance and outcomes of our provider engagement and growth programs, shaping how they run, surfacing risks early, and making the calls that keep them on track.. You will keep a real-time pulse on how specific programs, initiatives and maturity stages are performing, translate that into concise reporting for leadership, escalate issues before they become blockers, and be comfortable picking up the phone with practice staff and providers when the team needs an extra set of hands for outreach or follow-up.

As a  Senior Market Operations Program Manager, you will:

  • Program ownership: Own the end-to-end operating model and outcomes for provider engagement and growth teams programs, initiatives and roll outs, including timelines, milestones, dependencies, and whether the program delivers their intended results. Design how these workstreams run, keep them aligned and moving, and make the calls needed to hit the outcomes leadership is counting on.
  • Lead cross functional teams and drive progress through influence, data and timelines. 
  • Analytics & leadership decision support: Track program status and performance through various data sources. Translate the numbers into concise, on-cadence updates for senior leadership. Provide recommendations for the priority calls, tradeoffs, and mitigations leadership should make, and influence where the programs focus next.
  • Issue flagging & escalation: Spot risks and blockers early and route them to the right owner. Maintain clear escalation pathways so decisions happen fast and the right stakeholders are looped in at the right time.
  • External stakeholder engagement: Fill gaps as needed on growth and provider engagement work, including initial outreach to practices and providers, follow-up on engagement touchpoints, and direct conversations with practice staff and providers to keep momentum.
  • Cross-functional coordination: Partner with market operations, growth, provider engagement, analytics, and commercial teams to align programs on strategy and direction. Lead through influence across teams you don't own, drive decisions to resolution, and hold the group accountable to outcomes.
  • Operating model & process design: Design the operating model and playbooks that let provider engagement and growth programs scale, setting the standards and practices other teams follow rather than only documenting what already exists.

Success in this role looks like:

  • First 30 days: You’ve built a working understanding of the in-flight provider engagement and growth programs, met the key internal and external stakeholders, and mapped how status, risks, and decisions currently flow to leadership.
  • By 60 days: You’ve stood up a consistent reporting cadence for senior leadership, taken ownership of 2–3 in-flight programs, accountable for their outcomes and making the priority calls to keep them on track, while  closing gaps in provider outreach and follow-up directly where needed.
  • By 90 days: Leadership has a reliable, weekly view into program health. Escalation pathways are documented and being used. Providers and practice staff hear from someone consistently, even when the primary owner is unavailable.
  • Longer term: You’ve become the operating spine for CPH’s provider engagement and growth programs, the person leadership trusts to set priorities, shape how programs run  and what needs to move next.

You should get in touch if:

  • You’re exceptionally organized and are energized by keeping many moving pieces on track without losing detail.
  • You have experience building and monitoring physician network programs and/or clinical operations
  • Healthcare or value-based care is a must, and PMP certification or an MBA is preferred.
  • You’re fluent with data, comfortable pulling, interpreting, and communicating program metrics from various tools like CRMs, and turning them into clear narratives for senior leaders.
  • You have strong judgment on when to escalate, and you’re comfortable pushing an issue upward when it needs attention.
  • You’re confident and warm with external stakeholders, able to pick up outreach to a practice, follow up with a provider, or run a check-in call and represent CPH well.
  • You bring 5+ years of program management, operations, or client-facing experience.
  • You thrive in a fast-moving, early-stage environment where priorities shift and speed matters.

Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.

A reasonable estimate of the base salary range for this role is:
$107,000—$140,000 USD
