About the role

Sixty million Medicare seniors live with chronic disease. The care system sees most of them twice a year. Cadence is building the infrastructure to support them every day.

Cadence is a clinical AI company that delivers continuous, proactive care for older adults with chronic conditions like hypertension, heart failure, and diabetes. We pair patients with a dedicated clinical team, integrate deeply into health system EMRs and workflows, and use our Clinical Intelligence platform to monitor vitals, surface risk early, optimize medications, and close care gaps between visits. The result: patients engage with care 100x more than before Cadence, clinicians focus on judgment instead of administrative work, and Medicare saves $2.7M a week.

We operate as a full clinical care delivery organization, not a software vendor. Our clinicians work alongside health system partners, extending the reach of local primary care providers into patients' homes. We're now applying AI agents across these workflows – from alert review and medication titration to lifestyle coaching and care coordination – with clinicians always in control of clinical decisions.

The Role

We're hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey.

In this role, you will lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends. You will oversee patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider to optimize treatment plans, adjust medications, and determine the appropriate level of care. This role requires a high level of clinical autonomy, strong judgment, and close collaboration with Registered Nurses and Patient Success teams to deliver timely, patient-centered, and scalable remote care.

Licensure Requirements

  • Required: Active Compact Multi-State RN license
  • Required: Active NP licensure in at least two of the following states: TX, MI, NC, CA, PA, NJ, CT, AL

Licenses must be active and in good standing at time of hire.

Schedule Requirements

  • Monday through Friday schedule (Flexible scheduling available, with a core shift schedule of 9:00 AM to 6:00 PM in Mountain, or Pacific Time)

What You'll Do

  • Oversee the monitoring of patient vitals, symptoms, and labs in collaboration with our team of RNs and MDs.
  • Manage the patient's treatment plan and medications with an emphasis on initiating and titrating guideline-directed medical therapy (GDMT) for Heart Failure and other appropriate guidelines for Hypertension and Type 2 diabetes.
  • Respond to patient escalations identified via RPM including abnormal vitals, symptoms, and labs.
  • Lead regular virtual check-ins with patients to review labs and GDMT optimization and adherence. Our programs support both primary care offices as well as cardiology offices.
  • Ensure every healthcare interaction with Cadence is an exceptional experience that prioritizes the well-being of the patient and aligns with the goals of the health system.
  • Support internal growth efforts to help Cadence scale exceptional care delivery to patients with CHF and other chronic conditions including Hypertension and T2D.
  • Be instrumental in shaping the culture of one of the fastest-growing teams at Cadence.

What You Need

  • 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D) either in an outpatient or inpatient setting at a high-performing medical center.
  • Experience managing CHF with GDMT.
  • Active ANCC or AANP certification.
  • Master’s Degree as a Nurse Practitioner with the willingness to expand state licensure as Cadence adds new markets and partners. 
  • Home office setup with consistently stable Wi-Fi with strong upload and download speeds. A Wi-Fi speed test is required during the interview process to verify that these standards are met.
  • Ability to thrive in an environment founded on trust, autonomy, and direct communication & feedback. Love of owning of problems end-to-end. Passion for the patient/customer experience and systematically improving healthcare with digital innovation.
  • Experience working in a CHF bridge clinic or T2D clinic, a startup environment, and working with remote patient monitoring technology is preferred.
  • Comfortable using AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms) while maintaining clinical judgment as the primary guide for every patient decision.

Compensation

The anticipated salary range for this role is $125,000-$150,000 annually, which is based on role scope, level, and location. In addition to base compensation, this role may be eligible for incentive compensation as part of the overall total rewards package. This position is remote and based in the United States. Actual compensation will be determined based on factors including experience, skills, internal equity, and applicable law.
