Summary

Aledade is hiring a Implementation Specialist to join their remote team.

The Implementation Specialist will play a critical role in the successful onboarding of partner practices to Aledade’s tools, ensuring EHR systems are configured and optimized to enhance patient outcomes, reducing healthcare costs, and increasing operational efficiency. The role will require in-depth collaboration with practice stakeholders and the multidisciplinary Aledade team to facilitate the optimization of EHR systems and implement data-sharing strategies that align with Aledade’s population health solutions and strategic ACO initiatives. This individual will be responsible for managing client-facing projects from start to finish, providing ongoing coaching and training to practice staff, and ensuring the full adoption of value-based care models.

Primary Duties

Project Management

  • Independently manage multiple implementation projects at various stages, ensuring that EHR optimization, Aledade tools, and workflows are delivered on time and meet stakeholder expectations.

  • Communicate project progress to internal and external stakeholders, including practice leadership, IT staff, and front-line staff, ensuring full alignment with implementation goals.

  • Deliver clear, concise, and structured training content on complex material, making it accessible for diverse audiences (practice executives, IT teams, clinical staff).

  • Consult independent medical practices while providing detailed assessment and recommendations for full adoption of outcomes centric, value based care models.

  • Support/Coach partner practices through change management.

  • Influence the adoption of Aledade’s best practices and materials, driving engagement and commitment from practices throughout the implementation process.

  • Execute implementations at Aledade’s high service level standards.

  • Data Exchange and Integration

    • Ensure accurate data capture in EHR systems to generate quality measures and other reporting required for Aledade’s services.

    • Collaborate with practices to develop and implement data-sharing strategies, ensuring that practices are able to seamlessly exchange data with Aledade.

    • Work with the team to design integrations and interfaces that support data sharing and population health analytics, ensuring data flow aligns with Aledade’s reporting requirements.

    • EHR System Optimization

      • Conduct comprehensive assessments to understand how practices are currently utilizing their EHR systems and identify opportunities for optimization.

      • Collaborate with multidisciplinary teams and practice stakeholders to implement EHR solutions that support ACO initiatives and population health goals.

      • Perform technical discovery to understand practices’ technical landscape (which systems are used, where data resides) and explaining data flow/integration scenarios.

      • Assist practices in fully leveraging their existing EHR systems (athena, eClinicalWorks, Epic, etc.) for strategic ACO initiatives, including optimizing workflows and ensuring full adoption of technology for population health insights.

      • Assist practices with MIPS compliance, providing guidance on using EHR systems to meet reporting requirements.

      • Stay informed on EHR vendor system updates and the implications of these changes for population health management.

      • Provide Implementation/EHR Expertise

        • Fully manage the execution of implementation initiatives from start to finish.

        • Lead resolution of complex technical and operational challenges using creative problem-solving approaches.

        • Guide practices through sophisticated Aledade App configuration, enablement, and EHR optimization scenarios.

        • Identify common approaches, such as templates or customized reports, that practices can use to achieve specific aims, such as Medicare Wellness Visits, Transitions of Care, Advance Care Planning, or Risk/Hierarchical Condition Categories.

        • Identify existing or develop new high-quality, adult learning optimized educational materials that can assist practice staff in understanding how best to utilize their EHR system or other 3rd party systems to align with value based care principles.

        • Understand relevant EHR vendor system updates specifically related to population health management.

        • Develop and support EHR-system-specific user groups composed of representatives from Aledade’s ACO practices to identify best practices and training opportunities.

        • Serve as an in-house expert on at least 1-2 top-tier EHRs.

        • Leadership/Mentoring

          • Lead various program projects and provide thought partnership in designated areas while collaborating across internal teams.

          • Deep understanding and demonstrated ability to explain the business case behind initiatives to internal and external stakeholders.

          • Mentor and support team members, providing guidance on Implementation and EHR related tasks and initiatives.

          • Facilitate training sessions and develop educational material to enhance team member skills.

          • Think beyond their immediate team and contribute to making Aledade holistically better.
